8:00 am - 5:00 pm
$40.00 - $50.00 per hour
ESSENTIAL JOB FUNCTIONS
- Provide direct patient care in a home health setting.
- Evaluate patient functional needs and develop individualized treatment plans.
- Supervise and collaborate with Physical Therapy Assistants.
- Implement care plans and adjust based on patient progress.
- Document assessments, goals, progress notes, and discharge plans.
- Educate patients and families on home exercise programs.
- Communicate effectively with care teams to ensure optimal outcomes.
- Maintain compliance with safety, infection control, and quality standards.
- Promote community awareness of physical therapy services.
- Participate in ongoing professional development and staff education.
JOB REQUIREMENTS
- Graduate of an APTA-accredited Physical Therapy program.
- Current license from the Texas State Board of Physical Therapy Examiners.
- Home health and geriatric care experience preferred.
- Ability to lift, position, and transfer patients.
- Requires frequent standing, walking, bending, and kneeling.
- Strong interpersonal, communication, and documentation skills.
- Ability to care for patients across a variety of age groups and physical needs.
